The audit gets done twice
An auditor takes notes on site and writes the report in the evening. The same observation passes through twice, in two formats, and whatever falls between them goes unnoticed until the accreditation is reviewed.
The note and the report are separate things
A notepad, a Word template and a spreadsheet. The observation is written on site and rewritten afterwards, and every rewrite is a chance to lose something.
Traceability is built after the fact
Which requirement was covered by which question, and what evidence did the auditor see? The chain sits in the head of the person who was there, not in the system.
Every auditor works slightly differently
Without a shared structure, two audits against the same standard become two different documents, and nonconformities cannot be compared over time.
