Describe the client's processes and link the requirements of the standard to them. The scope is written on the assignment and follows through to the report.
Team and schedule

This is the part the auditor uses most. It is built for writing in, not for reading in.
Observations on site
Write down what you see while you see it. The observation is linked to process and requirement in the same step, without a detour via a notepad or a spreadsheet.
Questions and notes
The questions you asked and the answers you were given stay in their own wording. The assessment can be read against what was actually said.
Evidence and references
Point out which documents and procedures you examined. The reference sits on the observation, not in a separate list.
Coverage in real time
See which clauses of the standard are covered and which remain. The gap is visible while you are still at the client.
Several auditors at once
The team works in the same assignment and sees each other's observations. The lead auditor follows the work without having to ask.
